Guidelines:

• The plan applies only to services provided at North Georgia Smiles.
• The plan fees are non-refundable.
• The plan is intended for patients without dental insurance. Patients cannot use both dental insurance benefits and discounts from the plan.
• The plan cannot be transferred to another patient.
• Use of Care-Credit to pay for the plan and finance treatment will require a convenience fee. Please see the front office for details.
• For patients with gum disease, Periodontal Maintenance visits instead of regular cleanings will be covered for 2 of those visits per year, and will receive 20% discounts on any additional Periodontal Maintenance visits for the year.
• Procedures and products not eligible for discounts include: In-Office Prescriptions, Eletric Toothbrushes, and Invisalign.

We designed the North Georgia Smiles Membership Plan for patients without dental insurance. It makes excellent dental care accessible and provides generous discounts over regular fees.

The Membership Plan Offers:

• No yearly maximums
• No deductibles
• Immediate eligibility (no waiting periods)
• No claim forms
• No pre-authorization requirements
• No pre-existing condition exclusions

Plan Fees

Individual annual plan fee is $450 ($425 for each additional family member).

Since our exclusive Membership Plan is not insurance, all applicants are guaranteed acceptance.

You will not need a membership card – your plan is effective as soon as you enroll.
